UzObi, Inc. values your privacy and is committed to protecting all Personal Data (defined below) obtained during the use of its website, web platform, and other services (collectively referred to as the “Platform”), including Protected Health Information and Personally Identifiable Information. UzObi, Inc. is a Delaware corporation having its address at 14033 Commerce Ave. NE, Suite 300, PMB 316, Prior Lake, MN 55372 (“Company”, “We”, “Us”, “Our”, “UzObi”).

For the purpose of this Privacy Policy, the term “User(s)” and wherever context requires “You”, “Your”, “Yourself” shall mean any natural legal person who accesses or uses the Platform in any way whether through a user account or without a user account, and in the event that a natural person is representing a business entity, reference to such terms shall also include such business entity.

This Privacy Policy (“Policy”) describes how We collect, use, and disclose information that We obtain about visitors to and users of our Platform and other associated applications, products, and services managed by UzObi that reference this policy. The Platform has been developed to allow You to communicate with providers, hospital systems, healthcare personnel, caregivers, and/or other authorized third parties about Your care needs and Your healthcare journey. UzObi does not provide any medical or legal advice whatsoever at any time.

Personal Data means information that (either in isolation or in combination with other information held by UzObi) enables you to be identified or recognized directly or indirectly, including but not limited to Protected Health Information and Personally Identifiable Information. Protected Health Information refers to personal health information found in medical and billing records. This includes information written, verbal, or electronically recorded that is created and received by UzObi and relates to past, present, or future physical or mental health conditions and/or health services. UzObi cares about the privacy and confidentiality of Personal Data, Protected Health Information, and Personally Identifiable Information. Personally Identifiable Information refers to data relating directly or indirectly to an individual, from which the identity of the individual can be determined.

To deliver services, UzObi may create a record of the information You provide and the services You receive from Us. This record may contain Your identity information, health history, symptoms, examination and test results, diagnoses, a plan for Your future care or treatment, and any and all information related to the same. UzObi records that contain your information will be maintained in a confidential manner and as required by law. UzObi, and all of its employees, agents, representatives, and volunteers follow the privacy practices described in this Notice.

Collection of Personal Data

We may collect the following categories of Personal Data about Platform users/visitors, clients, prospective clients, suppliers, and other third parties:

Basic data: name, geographical information, contact details, health plan beneficiary numbers, and other information defined as Protected Health Information (PHI) and Personally Identifiable Information (PII).

Registration data: subscriptions, registrations, downloads, usernames, and passwords.

Usage data: data You have provided to Us as is necessary for a specific service We are providing, technical data relating to Your interactions with the Platform, client and user feedback, invoicing details, transaction data, and payment history.

Job applicant data: data provided by job applicants or others on the Platform or by other means in connection with employment opportunities.

Device data: Computer Internet Protocol (IP) address, Unique Device Identifier (UDID), operating system, screen size, and other data linked to a device.

Marketing data: demographic data, user behavior data such as session length, bounce rate, and pages visited, product interests and preferences, survey information, cookies, and browser data.

Compliance data: government identifiers, personal identifiers, identification documents, and due diligence data.

We collect data from a number of sources, either directly from the data subject, or from clients, colleagues, and publicly available sources. We also collect information You choose to provide to Us when You complete any “free text” boxes in our forms. Please note that the information above relates to data categories and includes but is not limited to the examples provided and may be collected under multiple categories at any given time.

Our Platform may include third party plug-ins or widgets that may provide information to third parties about your interactions with the Platform, even if you do not click on or otherwise interact with the plug-in or widget. The information is transmitted from your browser and may include any of the data categories listed above. We are not responsible for the privacy policies or data practices of third-party sites or their plug-ins or widgets.

Use of Personal Data

UzObi may collect Personal Data (including emails, phone numbers, addresses, etc.), Protected Health Information, and/or Personally Identifiable Information to perform our obligations in offering services, direct marketing, and the Platform, to protect the security and functionality of our Platform and technology systems, to make our Platform more intuitive and easy to use, to provide relevant marketing, to address compliance and legal operations, to consider individuals for employment or other contracting opportunities, and for the management of employees and contractors. Sharing of Personal Data

Except as otherwise described in this Privacy Policy, We do not sell, rent, or share your Personal Data with third parties. We may share your Personal Data in the following circumstances:

Vendors and service providers: To assist Us in meeting business operational needs and to perform certain services and functions Your Personal Data may be shared with third-party providers of hosting, email communication and support services, analytics, marketing, advertising, employee background checks, administrative and technical services (including Amazon Web Services and Google). These parties may access, process, or store Personal Data in the course of performing their duties for Us.

Legal Requirements: If required by law pursuant to valid legal process, applicable regulation or in the good faith belief that such action is necessary to (i) comply with a legal obligation, (ii) protect and defend Our rights or property, and those of You or others, (iii) act in urgent circumstances to protect the personal safety of You or others, or the public, or (iv) protect Us against legal liability.

Internal Business Departments: For the delivery, personalization, and improvement of services and the Platform. This may be necessary to satisfy local, state, and federal laws and regulations or in furtherance of Our legitimate interest in developing, providing, and improving services and the Platform.

The Purpose of this Notice

This Notice describes the general uses and disclosures that We make with Your Personal Data, certain rights that You have, and legal duties that We have with respect to such information. UzObi has developed policies, created procedures, and taken other steps to help keep Your information confidential. This Notice gives a summary of those steps, explains Your privacy rights, and shares phone numbers and addresses you can use to ask questions or make requests.

Our Duties 

We are required by law to:

  1. Make sure any health information and Personal Data that identifies You is kept private and secure;

  2. Give you this Notice of Our legal duties and privacy practices with respect to Your health information and Personal Data; and

  3. Follow the terms of this Notice as long as it is in effect. If We revise this Notice, We will follow the terms of the revised Notice as long as the revised Notice is in effect.

 

Your Rights

Your Medical BLUEPRINT™ is the property of UzObi and will be created, stored, and maintained with the utmost respect and security. You have the following rights, with certain exceptions, regarding the information that is created about You by UzObi.
 

  1. A copy of this Notice.

  2. A copy of Your records. You may obtain a physical copy of your Medical BLUEPRINT™ and Your medical records in various formats. Please visit the Platform to print a copy of Your Medical BLUEPRINT™ at any time during your subscription. You may also request a copy of Your Medical BLUEPRINT™ at any time by emailing records@uzobiinc.com.

 

Use and Disclosure of Health Information Electronically

We may use an electronic health record system to manage and/or store Your health information. We may create, receive, maintain, and disclose Your health information in electronic format for the limited purpose of providing services.

We may communicate with You through email. Emails or other electronic communications may not be encrypted or secure and could be read or otherwise accessed by another person or organization. We will assume that You understand these risks if You initiate electronic communication with Us or agree to receive communications from Us via email.

 

Authorization for Disclosures

We will not use or disclose Your data, except as described in this Notice, unless You authorize Us to do so. We will obtain Your authorization prior to electronically disclosing your health information. You may give Us written permission to disclose Your health information to anyone for any reason. If you grant authorization, We will make Your Health information available electronically as directed. UzObi may use electronic or other means to complete your request for authorized disclosure. You can revoke authorization through the Platform or at any time in writing. If You revoke authorization, UzObi will stop the disclosure of any information under the previous authorization. However, UzObi will not be able to remove or retrieve any information previously disclosed with proper authorization.

Sensitive Information

Alcohol and drug abuse information has special privacy protections. We will not disclose any information identifying You as being a patient or provide any health information relating to substance abuse treatment unless You consent in writing; a court order requires disclosure of the information; medical personnel need the information to meet a medical emergency; qualified personnel use the information for the purpose of conducting scientific research, management audits, financial audits, or program evaluation; or it is necessary to report a crime or a threat to commit a crime, or to report abuse or neglect as required by law. 

 

Future Communications

We may use or disclose Your information to communicate with You via newsletters, alerts, mailings, or other means regarding Your health benefits, Our services and/or other initiatives in which We participate. To opt-out of future communications please visit Us at info@uzobiinc.com.

Business Partners

We provide some services through contracts with vendors, business associates, and third-party providers. When receiving these services, We may disclose Your information to obtain the services needed. However, We ensure that no one will have access to Your information unless necessary to perform said services and We will maintain all legal requirements and privacy policies as described in this Notice.

Changes to our privacy policies

We reserve the right to change the terms and conditions of our disclaimers and Privacy Policies and We will post any changes on the Platform. You agree to review all disclaimers and policies each time You use the site, so You are aware of any modifications. By accessing and using the site or other offered services, You agree to be bound by the terms and conditions of the legal disclaimer, medical disclaimer, chat notice, Terms & Conditions, and Privacy policies in effect and as posted on this site at the time of your access or use.

Children’s Privacy

UzObi is committed to protecting the privacy of children. We will not knowingly collect, use, or disclose personal information from any person under the age of eighteen (18). Use of services and the Platform is limited to users and visitors over the age of eighteen (18) years old.
