Founding partner pilots open the week of November 30, 2026. UzObi is selecting a small founding cohort of physician practices, in primary care and in pulmonary and critical care medicine, to run Sanctuary live with their patients.
The pilot costs your practice nothing
For 90 days your practice receives Sanctuary, onboarding, training, and measurement at no charge. Your physicians hold the advance care planning conversations inside the relationships they already have, and your practice bills and keeps 100% of the advance care planning revenue (CPT 99497 and 99498, roughly $165 per completed conversation).
What we bring
The platform, member authoring and 54+ Care Guides, a training session for your team, and measurement built on one question: was the patient's voice documented and used at the moment of care. We establish your ACP baseline with you first, so the pilot is judged against your own numbers.
What we ask of you
A named clinician champion, a patient navigator as our day-to-day contact, and the staff time to run real conversations with real patients.
After the pilot
A fixed fair-market subscription, independently valued, never a share of what you bill. Founding partners receive preferred terms.
Where we are, honestly
Sanctuary is in live beta. It is designed to the standard, not yet certified, and no EHR integration is built; today guides reach the chart by print and placement, with direct delivery on our roadmap. We would rather tell you that here than have you discover it later.
