From conversation to Blueprint to care
A patient enters Sanctuary and, over a few guided sessions, writes their plan, in their own words. The result is their Medical Blueprint: a single living document that captures who they are and what they want, which they own and can revisit as life changes. From the Blueprint, Sanctuary produces Care Guides, each written for a specific reader on the care team and which the member publishes and shares with the people they choose. The patient writes once. Everyone who cares for them reads the version that fits their role.

Capture. The patient writes it down in Sanctuary, at home, at their own pace.
Translate. The Medical Blueprint becomes a Care Guide for each role on the team.
Share. The member chooses who sees each guide and can change their mind at any time. We're building toward delivering guides into the records clinicians already use.

Seven rooms of your plan
Â
Sanctuary's guided sessions cover the parts of a person a chart leaves out. Across seven dimensions, a patient records who they are, what they value, what a good day looks like, what they fear, who decides with them, and for them when they can't, what they want when they can no longer say so, and how they want to be treated along the way.

What the care team sees
One Blueprint produces many guides. Each is written for its reader and carries only what that reader needs.

Care navigator
a practical guide to what matters day to day, what the patient needs explained, and who in the family is involved.
Family
a plain-language version of the Blueprint, so no one has to guess what Mom would have wanted.
Every guide links back to the member's own words, so a reader can always see the source. Guides are published and shared by the member; delivering them into the medical record is on our roadmap.
Where Sanctuary is going
What's live today is the proof of concept: a member writes their plan, and the people who help them get a guide written for their role. It's the foundation, and it's deliberately not the end.
The platform we're building is fully AI-powered, with messaging built in. The Medical Blueprint stops being a document and becomes a living one: it notices what has changed, prompts the member to update it, reaches the right person in the village at the right moment, and carries the member's wishes into the room when they can't. Think of a plan that advocates for you, not one that waits to be read.
We're raising capital to build it. The beta and the founding-partner pilots exist to prove the concept, so that the next version is built on evidence, not assumptions.
Clinical Consultation
We provide expert strategic advisory for medical groups intent on embedding customized Medical Blueprints into their primary workflows. Our methodology guarantees that all professional clinical standards are addressed while grounding every care choice in the authentic perspective of the members in our founding-partner pilots.
UzObi, Inc. developed Sanctuary to streamline provider workflows and bolster recovery metrics through transparent, organized dialogue. We collaborate with your leadership to adapt these tools for unique diagnostic settings, prioritizing the members in our founding-partner pilots in every transition.
This professional service focuses on enterprise-level integration and strategic coordination. Sanctuary helps the people who care for you understand your wishes. It is not medical advice, and it does not replace a health care proxy, MOLST/POLST, or a living will.
Your voice. Your care. Your way.
