Sanctuary Terms of Use
For the Sanctuary pilot · Version pilot-terms-1.1 · Effective November 5, 2026
The short version
-
Sanctuary is a place to write down what matters to you about your care. You are the author of your plan.
-
This is a small pilot. Some things are still being built, and we will tell you what changes.
-
Sanctuary is not a legal document, not medical advice, and not monitored. For urgent help, call your health center, or 911 in an emergency.
-
You can stop at any time and delete your account.
1. What Sanctuary is
Sanctuary is made by UzObi Inc., a Delaware corporation ("UzObi", "we", "us"). It helps you write your own care plan and choose who may read it, so the people who care for you can act confidently on your behalf. By using Sanctuary you agree to these terms and to our Privacy Policy (uzobiinc.com/sanctuary-privacy).
2. The pilot
Sanctuary is offered as a pilot and may have errors or interruptions. To the extent the law allows, we are not liable for indirect losses from using it. Nothing in these terms limits a promise in our Privacy Policy or any right you have under law.
11. Ending
You can stop using Sanctuary at any time and delete your account in Settings. We may end your access if you break these terms, or when the pilot ends, and we will tell you first wherever we can.
12. Changes
If we change these terms in a way that matters, we will publish a new version here and tell you in Sanctuary before it applies.
13. Governing law
The laws of the State of Delaware govern these terms, without taking away any protection the law of the state where you live gives you.

A community health center ("your health center") invites some of its patients to try Sanctuary from December 2026 to about March 2027. Because this is a pilot, some features are unfinished and things may change. We will tell you in Sanctuary before a change that affects you, and before the pilot ends. When it ends, you can download your plan.
3. What Sanctuary is not
-
It is not a legal advance directive, health care proxy, living will or medical order. Writing something in Sanctuary does not make it one. Those documents have to be made the way your state's law requires.
-
It is not medical or legal advice, and it does not replace your doctor, your care team or a lawyer.
-
Sharing a guide with someone does not give them legal authority to decide or speak for you.
4. Urgent help
Sanctuary is not monitored. No one watches what you write in real time, and your care team may not read it before your next visit. For urgent help, call your health center, or call 911 in an emergency.
5. Your account
You must be 18 or older. Your account is yours alone. Someone you trust, such as a community health worker, may sit beside you while you set it up, but they never sign in as you, and the choices are yours to make.
Keep your sign-in safe. If you think someone else has used your account, email privacy@uzobiinc.com.
Your invitation card can be used once. Please do not give it to anyone else.
6. Sharing and your care team
If you choose to share with your health center, the care team members named for this pilot can read the care guides you share, and each read is shown to you. You can stop sharing at any time. What your health center has already read, printed or saved becomes part of your record there. Details are in the Privacy Policy.
7. Text messages
-
Program: Sanctuary account messages from UzObi Inc.
-
What you receive: sign-in codes and short account notices, only if you ask for texts in Sanctuary. No health information and they do not name your health center.
-
How often: message frequency varies, up to 10 messages a month.
-
Cost: message and data rates may apply. UzObi does not charge for texts.
-
Stop: reply STOP at any time to stop texts. You will get one message confirming you have opted out. You can also turn texts off in Settings.
-
Help: reply HELP, or email privacy@uzobiinc.com.
-
Carriers are not liable for delayed or undelivered messages.
-
Privacy: Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. Text-messaging opt-in data and consent will not be shared with any third parties. See the Privacy Policy.
8. Your words are yours
You own what you write. You let us store and display it only so Sanctuary can work for you and for the people you share with. Where Sanctuary suggests wording from its own library, it is marked, and you decide what stays.
9. Using Sanctuary fairly
Please do not misuse Sanctuary, try to get into anyone else's account, interfere with how it works, or use it to harm anyone.
